Your First Visit
On your first visit bring your completed forms and meet Sandy. If you have not downloaded and completed the forms online use the button below to complete. Copies are available at the clinic. Expect to spend 15-20 minutes filling out our forms. After you have checked in with Sandy, you will meet with Mladenoff or Dr. Chip, who will take a detailed history of any and all of your concerns.

We have 3 different types of initial patient beginnings. Based upon your medical history the Doctors will; determine the appropriate level of exam that will best suite your needs.
Please bring any lab work or x-rays including dictated reports that you currently have pertaining to your current or past significant issues with you to your first appointment.
The REGULAR EXAM
is our shortest exam and takes about an hour with Heart Rate Variability to assess your stress system, an Acugraph (ElectroMeridian Imaging). This if followed with a baseline of your vital (height, weight, BP, pulse oxygen saturation).
The COMPLETE EXAM
begins with prenosological computerized testing including Autonomic Nervous System Assessment, Brain Balance Tracking, ElectroMeridian Imaging and Zyto biochemistry scan. These tests reveal the status of the nervous sytem as a whole and are essential in developing your personal plan of care. This exam is our middle test taking about an hour and a half.
If you are seeking treatment for head trauma, concussion or a more complicated neurological case we will perform a TOTAL COMPREHENSIVE EXAM. During this exam you will receive all of the procedures and protocols in the Complete Exam with additional QEEG Brain Mapping and the RightEYE vision examination. These additional exams are non-invasive and provide windows to the function of different areas of your brain. We are then able to isolate the problems you are having related to any dysfunction. This examination takes about two and a half hours.
The final step to all three exams is to perform the hands-on- physical exam which includes a combination of functional neurological, orthopaedic, chiropractic, traditional Chinese medicine and applied kinesiology muscle testing.
After reviewing and analyzing the pronosological testing, physical examination results, the Doctors will develop and discuss a plan of care. Sometimes more significant testing is needed to be performed that is not available at our clinic. Dr. Mladenoff or Dr. Chip will explain what tests they will order and where they are available.
A second appointment will be scheduled to review your examination and test results along with treatment recommendations and a plan of care.

Payment Options
The Mladenoff Clinic believes in the value of wellness care for you and your family and have maintained a pricing structure that is affordable. Our office is an out-of-network provider for all insurance programs. The patient makes payment to the clinic as the service is rendered. All of your insurance paperwork will be done for you by the clinic. We are now a paperless compliant clinic, all of your claims will be submitted electronically for your reimbursement. If there is any insurance coverage, you will receive a check from your insurance company. If you have any further questions please call our office directly 913-491-1071.
